NEW BREMEN — OSHA is investigating a fatal industrial accident Monday afternoon at Crown Equipment’s Plan 4 operations in New Bremen, Auglaize County.
The worker, 49-year-old forklift operator Travis Temple, was struck by a lift truck about 2:15 p.m. and died later at St. Rita’s Medical Center in Lima, according to a statement from Randy Niekamp, vice president of human resources. Temple had been with Crown for more than seven years, Niekamp said.
