An initial investigation showed that Holly Rogers, 34, was driving a 2004 Toyota Sienna eastbound on Hollansburg-Arcanum Road.
The Darke County Sheriff’s Office said she failed to yield the right of way while crossing the intersection into the path of a 2024 Kia van going northbound on State Route 503.
Jake Sheard drove the van and hit the Sienna on the passenger side, and it came to a rest in the grass. The van remained on the road.
Medics transported Sheard Wayne Hospital and two juvenile passengers to Children’s Hospital with non-life-threatening injuries.
The crash remains under investigation.
