The Columbus Division of Fire, Occupational Safety Health Administration, National Chemical Safety Board, and Ohio EPA found that the explosion was caused after an agitator used to cool products stopped working causing a chemical reaction.

As the Chemicals began to mix, heating and pressurizing the tank, it resulted in a highly flammable gas leaking into the kettle room.

After the incident community members with the North Central Area Commission and other organizations have demanded answers and accountability, WBNS reported.
