URBANA — The Urbana Police Division and the Occupational Safety and Health Administration are investigating an industrial death at the Rittal Manufacturing facility in the city, according to police.

Fire crews responded to the facility around 10:40 a.m. Thursday after receiving a report of a man with his head stuck in a piece of machinery, according to initial dispatch records.
Details about the victim and how the incident may have happened were not immediately available.
OSHA has up to six months to complete its investigation, issue any citations or propose any penalties if any violations of health and/or safety standards are found, a spokesman told News Center 7.
A search of OSHA records online show no inspections at the facility since at least 2015.
