Collinsworth pleaded guilty to two counts of aggravated vehicular assault in May.

The other two counts, as well as the two counts of operating a vehicle under the influence of alcohol were dismissed, records show.

Collinsworth’s driver’s license was also suspended for two years.

The Jeep was then struck by another vehicle, driven by an 80-year-old woman, the crash report stated.
