UPDATE: Man killed in head-on crash involving semi in Darke County identified

DARKE COUNTY — A man driving a cargo van was ejected and died from his injuries after he crashed head-on into a semitrailer on U.S. 127 in Darke County Wednesday morning, according to investigators.

Yassin Mohamed Ishaq, 44, of Kansas City, Missouri was identified as the man who died.

Emergency crews were called the crash on U.S. 127 about a mile north of state Route 47 around 8:40 a.m. Wednesday.

Ishaq was the driver of a black 2021 Dodge Sprinter van.

An initial investigation found that the van was traveling north on U.S. 127 just south of Brown Road when it crossed the centerline and crashed head-on into a white 2018 Volvo tractor-trailer traveling south.

The semi then traveled off the right side of the roadway and struck a tree.

The driver of the semitrailer, who was identified as Zachary Garland 28, of New Madison, was treated and released at the scene.

Potential factors in the crash were still under investigation however weather does not appear to be a factor and deputies said road conditions were dry when the crash happened.

U.S. 127 was shut down for hours but reopened around 3:15 p.m., according to the Ohio Department of Transportation.
